Reporting and Learning Systems

Patient Safety reporting and learning systems are systems with the potential to improve patient safety through the analysis of reported adverse events/ incidents, dissemination of recommendations for system improvements, and implementation of preventative practices. This is achieved while maintaining a system-wide emphasis on seeking and understanding the lessons that can be learned. These systems can be divided into the following categories:

A National system is a system concerned with or applicable to or belonging to an entire nation or country. Feedback from the system can be based on reports from regional and/or local systems.

A Regional system is a sub-national system that operates inside a geographic defined field. Feedback from the system is entirely based on data from the region.
